ActiveHealth Management
Vermont Managed Care has partnered with ActiveHealth Management to provide Disease Management for Fletcher Allen Preferred subscribers and their dependents.  Disease Management is available for many types of conditions including diabetes, asthma, heart disease, arthritis, gastric reflux disease and many others.
The program is called Informed Care Management and is completely voluntary.  If you qualify for one of the programs offered through ActiveHealth Management, you will be receiving a letter or phone call inviting you to participate.  Please remember that if you do not wish to participate, you must decline the invitation.  This will avoid additional unwanted contacts.

Case Management
Vermont Managed Care has 7 Certified Nurse Case Managers on staff that are ready to assist you or your family members with complex health care needs, and helping you make your way through the health care system.
VMC Nurse Case Managers can help you with many issues such as:

  • Pre-Authorizing needed services
  • Coordinating home care
  • Coordinating multiple health services
  • Coordinating community services
  • Facilitating medical equipment for the home
  • Finding needed medical services out of the VMC Network
  • Coordinating payment with other insurance coverage
  • Locating alternative funding sources
  • Resolving claims payment problems

Employee & Family Assistance Program (EFAP)
The Employee and Family Assistance Program is available to all FAHC Employees and is designed to assist employees and their families who may be experiencing personal, family, or work related difficulties. It provides quick, confidential access to counseling and other helpful resources.
Issues EFAP can help with:

  • Chemical Dependency
  • Communication Issues
  • Individual and Family Issues
  • Work Performance Issues
  • Change Management
  • Life crisis and grief
  • Parenting
  • Group Services: EFAP can also provide consultation and services to groups of employees who are dealing with issues such as the death or loss of a colleague, stress, intra-departmental concerns, or other crisis.
  • Emergency Financial Assistance is available on a limited basis through the LeRoyer Fund.

Maternity Wellness Program

Maternity Wellness Program Brochure

This VMC sponsored program is offered to all Fletcher Allen Preferred subscribers and their covered dependents.  The program is voluntary and provides enrolled participants with valuable information and additional benefits to help with the transition from pregnancy to parenthood.  The Maternity Wellness Program is also offered to prospective adoptive parents with materials geared just for them.
The program includes:

  • A brief health risk assessment
  • access to books and education materials at no charge
  • lactation support
  • reimbursement for educational classes such as birth preparation, infant/child CPR and many other choices
  • Case Management is also provided for those who need extra support.
